Permission To Use E-mail Form
The Tanglewood HOA Board of Directors has been moving toward increased electronic communication. This consent form gives the Assocation permissio to communication all notices via e-mail instead of using U.S. mail. This permission is intended to be broad and may include transmission of meeting notices, ballots, proxies, proposed and final annual budgets, financial statements and reports, audits, notices of annual regular assessments and special assessments, proposed and new rules and regulations, amendments to the governing documents, violation notices, hearing notices, information about my assessment account and other important Association documentation and information.

Owner Registration Form
For the purposes of safety, ability to vote in Association elections and to ensure that unit owners receive important notices from the Association regarding important matters, the Association has initiated an effort to update its Member List. It is important to have each owner indicate the correct names and addresses of the current owners of your unit(s). In some cases, our records show only one owner but the DuPage County tax records show multiple owners or a Trust as the owner.
In some instances, unit owners are renting their units and our records show only the unit address. Therefore, all official Association mail is being sent to the unit and you may not be receiving it unless your tenants give it to you. As a result, you may be missing out on IMPORTANT notices.

The authority to collect this information is found in your Association’s By-Laws, Article IX Officers and their Duties. Section 8 (c) The Secretary shall keep appropriate current records showing the members of the Association together with their addresses…”
Article VIII, Section 3. Notice of Meetings provides: “Written notice of each meeting of the members shall be given by, or at the direction of, the secretary or person authorized to call the meeting, by mailing a copy of such notice, postage prepaid at least ten (10) days, but not more than thirty (30) days, before such meeting to each member entitled to vote thereat, ADDRESSED TO THE MEMBER’S LAST APPEARING ADDRESS ON THE BOOKS OF THE ASSOCIATION, OR SUPPLIED BY SUCH MEMBER TO THE ASSOCIATION FOR THE PURPOSE OF NOTICE…”

Tenant Registration Form
FOR INVESTOR OWNERS ONLY:
If you own a unit in Greenbrook Tanglewood that is occupied by someone other than the owner, please complete the Tenant Registration form, in addition to the Owner Registration form. Additionally, for any lease executed on or after August 01, 2010, including prior leases that have been extended, please deliver to the above address a copy of the signed lease or if there is an oral lease, a memorandum of the lease.
The statutory authority for this requirement is as follows: The Illinois Common Interest Community Association Act, effective August 01, 2010, Section 1-35. Unit owner powers, duties, and obligations. (a) “ The provisions of this Act, the declaration, bylaws, other community instruments, and rules and regulations that relate to the use of an individual unit or the common areas shall be applicable to any person leasing a unit and shall be deemed to be incorporated in any lease executed or renewed on or after the effective date of the Act. With regard to any lease entered into subsequent to the effective date of this Act, the unit owner leasing the unit shall deliver a copy of the signed lease to the association or if the lease is oral, a memorandum of the lease, not later than the date of occupancy or 10 days after the lease is signed, whichever occurs first.”

Lease Rider Form

FOR INVESTOR OWNERS ONLY:
As part of the board’s efforts to promote Greenbrook Tanglewood as a crime free community, they have approved a lease rider. This rider must be incorporated into the lease of a unit in accordance with the Rules and Regulations of Greenbrook Tanglewood Homeowners Association, Inc. This means it must be included with all new leases or upon renewal of an existing lease. The rider requires both the owner and tenant to explicitly acknowledge that they, any guest, non-listed occupants, etc. are to abide by all respects to the provisions of the Declaration, By-Laws and Rules and Regulations of the Association, and the Village of Hanover Park Crime Free Lease Addendum and Related Ordinances and any failure by the parties to the lease or occupants to comply with the terms thereof shall be a default under the lease.

Ombudsman Policy
The board adopted a policy in compliance with the Condominium and Common Interest Community Ombudsperson Act. An owner may use the form to file a complaint against the association.
